[Caregiving Listserve] from the National Alliance for Caregiving
Last fall, the Institute of Medicine (IOM) convened a two day workshop on the future of home health and home-based models of care delivery. The Alliance was honored to participate in this workshop and to provide remarks on behalf of family caregivers through our President & CEO, Gail Gibson Hunt.
The workshop summary is now available free of charge from the IOM website. Highlights from the report include a discussion of how new models of home health should incorporate the following principles:
A need to focus on the sickest, highest-cost patients and the patient's level of function
The need for quality and outcomes measurement, better transitions of care, and better coordination of care
The appropriate use of technology, including information technology and telehealth
Support for family caregivers
Improved training for home health workers
Better connections between health care and social services
